What is Subcision for Acne Scars?
A rolling acne scar is not a hollow worn into the surface. Beneath it, fibrous bands left behind by old inflammation tether the skin to the tissue below and hold it down. That is why the depression persists no matter what is applied on top of it.
Subcision releases those bands. Under local anaesthetic, a fine instrument is passed beneath the scar and the tethers are divided, allowing the skin to lift, while new collagen forms in the space created. Bruising follows and settles over the following days. Not every acne scar is tethered, so the first task is establishing which of yours are.

Subcision is judged by feel, at a depth the eye cannot see. Performed without that judgement, it releases too little and achieves nothing, or too much and leaves firm nodules and bruising where a scar used to be. Bruising and swelling follow subcision and usually settle within a week, occasionally taking longer. The lift becomes apparent as that settles, and improves further over the months as collagen forms beneath the released scar. Most patients need more than one session. Some scars re-tether over time and require repeating, and ice pick or sharply defined boxcar scars will not respond to this at all. Your dermatologist will tell you which of your scars fall into which group.
